if the hypoglossal nerve is affected the tongue deviates to the diseased side due to crossing of the muscles
Right sided lesion of CN-XII (Hypoglossal)
This is due to the extrinsic muscles on the unaffected side pushing the tongue to the affected side. As the patient in this picture has their tongue deviated to the right upon protrusion, they have a right sided lesion.
